surgery1Chambers Health is a collection of related operations organized to provide you and your family with a coordinated approach to your care and well-being. Bayside Community Hospital, with its surgical, inpatient, and emergency services capabilities, among others, serves as the anchor of Chambers Health. Just as important are our Medical Clinics, located in Anahuac and Mont Belvieu, delivering medical, mental health and dental services. The most recent addition to the organization is The Wellness Center at Bayside, providing a variety of fitness and wellness programs, as well as serving as the host site for the organization’s therapy and physical rehab programs.

Chambers Health also provides a variety of other independent programs, hosted by the organization’s various operating units:

“Community Clinics”

The Community Clinic program was established in 2001 as a result of recognizing that Chambers County is just a bit spread out. The program provides the opportunity for those who do not have routine access to transportation to see a primary care provider closer to home. The Community Clinic program runs three times a year: in the Spring to check on allergies; the Summer, to help with those back-to-school physicals and shots; and Fall, to make sure everybody who wants one gets a flu shot. From the very tip of the County in Smith Point to our northern boundaries in Wallisville, there’s sure to be a Community Clinic close to you in the near future. “Women’s Health Program”

Begun in 2003 to address an unmet need in the community, Chambers Health’s Women’s Health program focuses on providing a specific set of services and tests for the Women of our communities. Growing in popularity on a daily basis, this program is now available at several of our sites. With significant grant support from the Houston Affiliate of Susan G. Komen for the Cure©, and in partnership with “The Rose” of Houston, the program provides mammography screening services as well as other tests to identify any issues or areas of concern. “Silver Gators Program”

Developed and designed to provide for the social and health related needs of the independent senior adults of the community, the “Silver Gators” program provides an opportunity to get out of the house and receive mental, physical & social stimulation. It is available to those in our community who are 60 years of age and older.  This unique and fun club was developed to help seniors take a proactive approach to wellness in a congregate setting.  Call 409.267.3700 for more information, or click here to download your application to become a “Silver Gator”.

“Assistance/Outreach Programs”

As a public hospital district, Chambers Health has primary responsibility; within the geographic boundaries of the district, for the delivery of care for the un/underinsured populations that call mid-County home. The organization accepts and supports that duty and responsibility by leveraging local resources against state and federal funds (grants) to provide a medical home for those who do not have health care insurance coverage or access to regular primary care services. Services can include regular office visits, laboratory and radiology testing, as well as some access to prescription drugs. There are requirements to participate in the program, so call ahead to (409) 267-4126 ext 61 and schedule an appointment with Maria Davila to see if you may qualify.

The Outreach Program at Bayside can also assist with identifying other state and federal programs that may be available to the applicant, and:

  • Pharmacy Assistance, through participation in direct-from-the-manufacturer programs, for those who qualify
  • Bayside Assistance programs, for inpatient and outpatient programs not covered by the SDI Program, based on need and/or financial circumstance.
